General Statement of Duties

The Team Lead works along-side the Residential Manager to develop, implement, and coordinate supports that address the needs and interests of consumers and assist them in safely reaching their personal goals for skill development and community integration.

Responsibilities

 

Supervises:

  • Day Program Support Staff

Essential Job Functions:

  • Creates and oversees implementation of daily schedule and activities in the day services program. Implements sound practices that result in the delivery of quality services to consumers.
  • Develops services structure and activities for each consumer in accordance with ISP and/or IDT.
  • Leads person-centered team of staff; educates staff on how to meet individualized consumer outcomes.
  • Maintains supportive relationship with each consumer.
  • Develops, coordinates and participates in all ISP meetings for consumers.
  • Ensures proper implementation of behavior modification plans for specific consumers. Informs Assistant Director of Day Services and Behaviorist when significant behaviors arise.
  • Works closely with nurse to strengthen liaison relationships with medical care providers and to ensure staff receive training by nurse as needed.
  • May communicate with families, guardians and residential staff. Responds to requests to make sure consumer needs are met.
  • Ensures services operate within budget including appropriate use of Substitutes. Administers and submits receipts for petty cash.
  • Effectively engages with staff to further the mission of the agency.
  • Reviews time cards of all direct reports every day and approves on a bi-weekly basis.
  • Conducts regularly scheduled supervision.
  • Ensures staff meet annual service goals and individual performance goals.

Qualifications

Required Skills & Qualifications:

    • Minimum High School Diploma/GED
    • Experience working with individuals with autism, hospice care, or senior care is a plus
    • 1-year experience working with developmentally disabled individuals
    • Computer literacy using Microsoft Word; familiarity with an electronic health record system preferred
    • Knowledge of behavior management techniques and strategies
    • Ability to lift and transfer individuals as needed
    • Strong communication skills to effectively interact with individuals, families, and other team members
    • Understanding of resident rights and ability to advocate for individual needs
    • Must be 21 years of age or older
    • Two (2) years experience working with persons with physical and/or developmental disabilities; 1 year experience as supervisor
    • Ability to understand and implement individualized supports

USD $53,000.00 - USD $53,000.00 /Yr.
